Problem-solving guides
8D Problem Solving Guide
Covers D1–D8 from team and problem definition through containment, occurrence cause, escape/detection weakness, corrective actions, verification, effectiveness review, prevention, and closure.
Use for: Customer complaints, repeat defects, serious nonconformances, and formal problem-solving responses.
Read the 8D guideRoot Cause Analysis Guide
Shows how to define the problem, identify possible causes, check the evidence, and reach an evidence-supported cause conclusion before selecting corrective action.
Use for: Defects, customer complaints, or process problems where several possible causes must be tested.
Read the RCA guideFishbone Diagram Guide
Shows how to organize possible causes by category and decide what evidence the team should collect before reaching a cause conclusion.
Use for: Team investigations involving method, machine, material, measurement, people, or environment.
Read the fishbone guide5 Whys Guide
Shows how to build and test a 5 Whys cause path, recognize when the chain is too narrow, and check each answer against evidence.
Use for: Focused investigations where one possible cause path can be tested step by step.

Non-Conformance Report Guide
Covers the requirement not met, actual condition, affected quantity, containment, disposition, correction, evidence, linked actions, and closure.
Use for: Nonconforming product, internal defects, supplier issues, customer returns, rework, scrap, or concession decisions.
Read the non-conformance report guideCorrective Action Guide
Explains the difference between correction, corrective action, verification of completion, effectiveness review, and closure.
Use for: Process changes that need an owner, due date, evidence, verification, effectiveness review, and a closure decision.

Fishbone diagramA fishbone diagram groups possible causes so the team can decide what evidence to collect and which causes to test.
Quality situation
Investigate one possible cause path

5 WhysUse 5 Whys to follow a focused cause path and check each answer against evidence.
Quality situation

Root cause analysisRoot cause analysis separates assumptions from evidence before corrective action is selected.
Quality situation
Record a defect or nonconforming product

Non-conformance report (NCR)An NCR records the requirement not met, actual condition found, affected quantity, containment, disposition, correction, evidence, and closure.
Quality situation

Which guide should I read first for a customer complaint?
Start with the 8D guide when the customer requires a formal D1–D8 response or the issue is significant, repeated or cross-functional. Start with the NCR guide when the immediate job is to record and control nonconforming product, then use the RCA or corrective-action guides as needed.
What is the difference between 8D, RCA, 5 Whys and fishbone?
8D is a structured D1–D8 response covering containment, cause analysis, corrective actions, verification, effectiveness, prevention and closure. RCA is the broader investigation used to establish why the issue occurred. A fishbone diagram maps several possible cause paths; 5 Whys follows one focused path and tests each answer with evidence.
Do I need a guide or a template?
Use a guide to understand the method and decide what evidence or action is needed. Use a template when you are ready to document the work in a standalone form, worksheet or report.
Managing Several Quality Records at Once?
Use the guides to choose the method and the templates to complete a standalone record. Join early access if your team is controlling actions, due dates, evidence, approvals, and customer responses across multiple quality issues.
