Choose the record that matches the job. Use an 8D for a formal D1–D8 response, an NCR for a requirement not met, a SCAR or CAR for a formal corrective action request, and the cause-analysis templates when the team needs to investigate before assigning corrective action.

Use this for a formal D1–D8 response. It covers the team, problem description, containment, occurrence cause, escape/detection weakness, corrective actions, verification, effectiveness review, and closure.

What to Check Before Using a Quality Template
The required fields depend on the record. Before using a template, check that it captures the information needed for the job and supports review and closure.
- Problem, defect, complaint, or requirement not met
- Affected part, process, customer, supplier, shipment, lot, or work order
- Record owner, date opened, and response due date where needed
- Containment, disposition, or correction where applicable
- Evidence such as measurements, inspection records, photos, emails, or supplier responses
- Cause investigation and evidence supporting the cause conclusion where required
- Corrective actions with owners, due dates, and verification of completion
- Effectiveness review, closure decision, and customer response or final report where required
Choosing Between a Template and Quality Issue Software
Use a Word or Excel template when one person can control the record, actions, evidence, and customer response through closure. Use quality issue software when several owners, changing due dates, approvals, evidence, and customer updates need to be controlled across multiple records.

What is the difference between an 8D template and a corrective action plan template?
An 8D template covers the full D1–D8 problem-solving response, including containment, occurrence cause, escape or detection weakness, corrective actions, verification, effectiveness, prevention and closure. A corrective action plan focuses on the actions, owners, dates, evidence, verification, effectiveness review and closure decision.
What is the difference between a 5 Whys template and a fishbone diagram template?
A 5 Whys template follows one possible cause path step by step. A fishbone diagram maps several possible causes before the team decides which paths to investigate with evidence.
